Members of the IoS £3.50

Non-members £7.00

Teachers Record Form

Here is a downloadable copy of the Teachers Individual Record Form in Word DOC format so that you can download a copy and edit it on your own computer if you wish. It is needed by the club to update it's records on Teachers, coaches and poolside helpers. Please return to the Teachers Secretary Gail Nuttall.
